Botox ® wrinkles

There are two main types of wrinkle:

• static wrinkles are linked to the melting of support volumes (dermal thickness, subcutaneous fat), which create furrows into which the skin eventually "falls". These wrinkles are treated by injection of a filler such as hyaluronic acid.
• dynamic wrinkles are linked to hypercontraction of the skin muscles, associated with a loss of elasticity in the tissues enveloping them (the SMAS or superficial musculoaponeurotic system) and the skin. These expression lines are treated with botulinum toxin, thanks to its muscle relaxant effect.

Botulinum toxin, or BTX-A, binds to acetylcholine receptors, enabling nerve impulses to contract muscle fibers. Once these receptors are saturated, the muscles can no longer contract: they relax and the skin becomes smoother. This blocking effect is progressive (the time it takes for the receptors to become saturated, i.e. 4 to 10 days) and transient (the rejuvenating effect lasts an average of 6 months, as the botulinum toxin is gradually broken down).

These dynamic wrinkles have the particularity of transforming emotions, especially on the upper face, giving a hard, severe, sad or tired look.

Dr Santini recommends Botox anti-wrinkle injections to remove dynamic wrinkles of the upper face (forehead wrinklescrow's feet at the corner of the eye, frown lines between the eyebrows...) and the associated negative emotions.
A successful anti-wrinkle injection means rejuvenating the face without surgery, simply by injecting it, while at the same time restoring natural, positive emotions, without giving a frozen or inexpressive appearance.

An initial aesthetic consultation with Dr Santini is necessary to objectify dynamic wrinkles and understand the patient's request.

They may be suffering from the negative emotions they unintentionally express, which is why it's important to understand them and determine their possible causes.

Dr. Santini's dual medical and aesthetic analysis focuses on the entire face and eyes, at rest and in action: she may ask you to assume different expressions to better appreciate the dynamics of your wrinkles and take medical photos.

There are few medical contraindications to Botox wrinkle injections:

• Pregnancy and breast-feeding, as a precautionary measure.
• Neuromuscular pathologies such as myasthenia gravis.
• Allergy to botulinum toxin or albumin.
• Precautions in the event of ongoing anti-coagulant treatment.

The anti-wrinkle injection session takes place at Dr Santini's surgery (75116 Paris), during a medical procedure without general anaesthetic.

The injection can be localized to a specific area, or be performed over the entire face, for a true medical facelift.

The duration is therefore variable, ranging from a few minutes to half an hour.

To make the procedure virtually painless, Dr. Santini always uses a combination of Botox and local anesthetic, and the injection is performed with a fine needle. An anesthetic ointment such as Emla can be added for the most sensitive areas.

Dr Santini may ask you to take on certain expressions (a stern look, an astonished look, etc.) to fine-tune the Botox wrinkle injections, depending on the negative expressions you wish to soften.

Dr Santini is always very careful to offer a natural rendering, which requires 3 conditions to be met:

• identify the skin muscles to be targeted, and the practical knowledge of an aesthetic surgeon is a real asset;
• identify the injection site (entry point, depth) ;
• choose the right dose to inject, so as not to freeze expressions with too high a dose.

Anti-wrinkle injections do not require you to take time off work, as any bruising or redness at the injection sites is not intense. They can be masked with make-up or foundation after 24 hours.

Dr Santini advises against any significant physical activity immediately after Botox wrinkle injections, as well as against massaging the treated areas.

The muscle-relaxing effect is progressive, revealing itself between 4 and 10 days, with a non-surgical rejuvenating effect lasting around 5 to 6 months. Positive emotions are restored, with a softer, less harsh, more cheerful and rested appearance.

It is usual to have a slightly shorter duration for the first injection, closer to 5 months than 6 months.
